Claims Center

CLAIMS PROCEDURES

YOU MUST CONTACT THE CLAIMS CENTER AT 1-888-250-4801, Monday – Friday: 8am – 8pm EST, Saturday: 9am – 5pm EST, PRIOR TO HAVING ANY REPAIRS PERFORMED RELATED TO THIS AGREEMENT OR ANY BENEFITS PROVIDED HEREIN. Any repair made without the prior authorization of Forever Clean Rewards will not be reimbursed.

To file a claim, please call the Claims Center at 1-888-250-4801, Monday – Friday: 8am – 8pm EST, Saturday: 9am – 5pm EST, and have Your membership information available, including Your Member Number, vehicle information, etc.

The Customer Service Representative (CSR) who answers Your call will ask You to describe the nature of Your claim, confirm Your membership information and possibly ask You to submit various documentation via fax, e-mail or postal service.

If your claim is for an Accident Expense Reimbursement, the CSR may instruct You to follow the below BENEFIT REIMBURSEMENT PROCEDURES for Accident Expense Reimbursements.

If Your claim is for a covered Exterior Paint Restoration or Headlight Restoration, the CSR may suggest, or help You choose, a local repair facility, provide You with a Claim Number and instruct You to have the repair facility contact the Claims Center at 1-888-250-4801, Monday – Friday: 8am – 8pm EST, Saturday: 9am – 5pm EST, referencing Your Claim Number.

If Your membership is valid and the description of damage meets the criteria for an eligible benefit, the CSR will provide the calling repair facility with an Authorization Number for a specified amount and authorize the covered repair. The repair facility is advised to fax or email a copy of the final invoice for payment to the Claims Center and is given the option to have payment issued to the repair facility via credit card or reimbursed to the customer via a check. It is the member’s responsibility to determine if the repair center will accept credit card payment from Us or if the repair facility will only accept payment directly from You. If You are required to pay the repair facility directly, please follow the below BENEFIT REIMBURSEMENT PROCEDURES for Exterior Paint Restoration and Headlight Restoration to obtain reimbursement for the authorized amount.

REQUIRED DOCUMENTATION:

  1. Copy of the paid repair order, indicating the covered repairs were performed, from a Forever Clean Rewards authorized repair facility. The repair order must include the date of repair and the repair facility’s preprinted name, address and telephone number. It must also include the covered vehicle’s year, make, model, odometer reading and an explanation of the repairs performed.
  2. Verification of Your current membership, including a copy of Your Benefits Page, which was provided upon your initial registration, and copies of credit card payments, if requested by Us.
  3. Any additional documentation required by Us.

IMPORTANT PROVISIONS:

  1. Any repair made without the prior authorization of Forever Clean Rewards will not be reimbursed.
  2. No reimbursement will be made and You will responsible for all repair costs if it is determined that the repair is not eligible or for any costs in excess of what is eligible for reimbursement.
  3. Reimbursements will only be made to the registered Forever Clean Rewards Member.
  4. Reimbursement requests will be processed within 20 working days of receipt of all Required Documentation.
  5. Reimbursement requests must be postmarked within 30 days of the date of the covered repair, as indicated on the repair facility’s invoice, to qualify for reimbursement.

To submit a claim for Accident Expense Reimbursement, please send the below REQUIRED DOCUMENTATION to:

REQUIRED DOCUMENTATION:

  1. Copy of each paid invoice for each expense for which You are seeking reimbursement. Paid invoices must be dated and include the business’s preprinted name, address and telephone number. Each invoice must also include a complete description of the goods or services provided and for which You paid. If related to the member’s covered vehicle itself, the invoice must also include the vehicle’s year, make, model and odometer reading.
  2. Verification of Your current membership, including a copy of Your Benefits Page, which was provided upon Your initial registration, and copies of credit card payments, if requested by Us.
  3. A copy of Your insurance settlement, including a copy of the final settlement details provided by Your insurance company and the final repair invoice from the body shop that performed the repairs.
  4. Any additional documentation required by Us.

  5. Reimbursement requests must be postmarked within 30 days of the date of the covered expense, as indicated on the providing business’s invoice, to qualify for reimbursement.

FRAUD AND MISREPRESENTATION

This Agreement is issued in reliance upon the truth of all representations made by You. No reimbursement will be made where You:

  1. Intentionally concealed or misrepresented any material fact;
  2. Engaged in fraudulent conduct; or
  3. Made a false statement relating to submitting a Benefit request. If You have concealed or misrepresented any material fact(s) concerning Your Membership, or in case of fraud, attempted fraud, or the false swearing by affecting any matter relating to this coverage, whether before or after the Benefit request, Your membership may be voided.
